Pharmacology:
Hexisol Handrub contains Chlorhexidine, a potent cationic chemoprophylactic agent with a broad spectrum of activity against gram-positive and gram-negative bacteria. It acts as both a bacteriostatic and bactericidal agent depending on its concentration. At high concentrations, it binds to negatively charged bacterial cell walls and extracellular microbial complexes, exerting a bactericidal effect. At lower concentrations, it disrupts bacterial cell osmotic equilibrium, causing leakage of potassium and phosphorus, thus acting as a bacteriostatic agent.
Dosage & Administration:
- Adults:
- Pre-operative surgical hand disinfection: Dispense 5 ml of solution, spread thoroughly over both hands and forearms, rubbing vigorously. When dry, apply a further 5 ml and repeat the procedure.
- Antiseptic hand disinfection on the ward: Dispense 3 ml of solution, spread thoroughly over the hands and wrists, rubbing vigorously until dry.
- Disinfection of patient skin: Prior to surgery, apply the solution to a sterile swab and rub vigorously over the operation site for at least 2 minutes.
- Elderly and Children: No special dosing regimes are required.
Interactions:
Chlorhexidine is incompatible with soaps and other anionic agents. Hypochlorite bleaches may cause brown stains on fabrics previously in contact with chlorhexidine solutions.
Contraindications:
Not suitable for individuals with known hypersensitivity to chlorhexidine. Such reactions are extremely rare.
Side Effects:
Occasional irritative skin reactions and extremely rare generalized allergic reactions have been reported.
Precautions & Warnings:
Avoid contact with the brain, meninges, middle ear, sensitive tissues, and eyes. Do not inject or use in body cavities.
Overdose Effects:
Symptoms may include pharyngeal edema, necrotic lesions of the esophagus, and elevated serum aminotransferase concentrations. Management involves gastric lavage with milk, raw egg, gelatin, or mild soap, along with appropriate supportive measures.
